Our Office
Where is your office located?
Our address is 2111 W Swann Ave. Suite 104 Tampa, FL 33606.
We share office space with Elevate Health, so you will see both of our names on the door. There is also a pink Muse flag outside of the front door, which is on Westland Ave.
Where should I park?
There is a free covered and uncovered parking lot right behind our building with entrances on Westland Ave. Street parking on Westland is also free anytime other than overnight from 6pm-6am.
Should I book a consultation first or can I do the consultation & treatment in the same day?
That is up to you! We have both options available when booking, so you are able to select just consultation or “consultation and treatment” for your appointment.
I've never had anything done and don't know where to start. What should I book?
Start with a complimentary consultation with any of our providers. We are all educated on all of the services that are offered at Muse and can help you start a treatment plan.
Do you take walk-ins?
No, we only work by appointment at this time. We do take same day appointments if you call or text us at 813-771-2061.
What is your cancellation policy?
We require a card on file to book an appointment because we have a no show/late cancellation or reschedule fee of $100. Any cancellations or reschedules within 24 hours of your appointment will be subject to the $100 fee.
Is a credit card required on file to book an appointment? Do you charge a deposit?
We do not charge a deposit for appointments at this time, but we do have a $100 fee for no shows or cancellations/reschedules within 24 hours of your appointment time.
